Dear friends,
This information is intended for all those who will join the International Creative Project for Children NucKids 2013 and their parents.
By May 27th, 2013 you should send the filled form with answers to the following questions to the e-mail addresses pronin@nuckids.ru and DASosulnikov@rosatom.ru:
1. Surname, first name, patronymic (example – Pronin Pavel).
2. Date of birth (DD.MM.YYYY, for example – 27.08.1975).
3. The series and number of the Russian passport (example – 22 02 208082). If you do not have a Russian passport yet, the series and number of the birth certificate (example – I-TN No. 547567).
4. Issuing authority, date of issue (for example, the Department of Internal Affairs of the Navashinsky district of the Nizhny Novgorod region, 26.06.2002).
5. The unit code (example – 772-039).
6. Registered address (for example, Navashino, Nizhny Novgorod region, Pochtovaya str., 1-122).
7. The series and number of the foreign passport (example – 70 0000000).
8. Issuing authority, date of issue (for example, the Ministry of Foreign Affairs of the Russian Federation, 23.05.2013).
9. Spelling of name and surname in the Latin alphabet, strictly as in the passport (example – PETROV ALEXANDER).
10. Passport validity period (expiration date, example – 07.08.2023).
Please fill in this form as an answer to the questions. If you do not have a passport, you should write about it and indicate the approximate date of its issuance. Please note: we do not need scans of these documents. We need the information in text form. We are ready to answer all questions by phone +7 985 4270777 (Pavel Pronin) or +7 499 9494746 (Denis Sosulnikov).
By June 10th, 2013 you need to send us the following documents by courier mail for Schengen visa application:
1. Application form (to be filled in Russian), you can download it here
2. Application form printed on both sides (the application form should not be filled in, both parents should sign it), you can download it here.
3. Two photos 35×45 mm (color, matte, on white background, face 80% of the photo, without corners and ovals). The photos must be new and not used for other visas.
4. A copy of the civil Russian passport (all completed pages shall be copied).
5. A copy of the birth certificate.
6. A foreign passport, the validity of which ends no earlier than March 2014. Two free pages must be available (strictly the original)
7. Certificate from the place of study (with the seal and signature of the responsible person). The certificate of the educational institution shall contain the address, phone number of the school and student’s class.
8. The consent of both parents, original and copy (both original and copy must be notarized).
9. Copies of Russian passports of both parents (all completed pages shall be copied)

We, Ivanov Ivan Ivanovich and Ivanova Maria Sergeevna, give our consent to the departure of our minor son Ivanov Peter Ivanovich, born on January 01, 1988, to Hungary and other Schengen countries in the period from 01.07.2013 to 20.08.2013, accompanied by the group leaders, Trapeznikova Anna Sergeevna, born 20.07.1961, (passport 4508 898675, issued on 26.12.2006 by the Troparevo-Nikulino Department of Internal Affairs of Moscow, subdivision code 772-039, registered at the address: Moscow, Michurinsky Prospekt “Olympic Village”, 19, apt.104) and/or Sosulnikov Denis Alexandrovich, born 01.12.1980, (passport 2203 076243, issued on 06.12.2002 by the Kanavinsky district Department of Internal Affairs of Nizhny Novgorod, subdivision code 522-002, registered at Nizhny Novgorod, Arkhangelsk str., 16, apt.50) and/or Pronin Pavel Vyacheslavovich, born 27.08.1975, (passport 2202 208082, issued on 28.06.2002 by the Navashinsky Regional Department of Internal Affairs of Nizhny Novgorod district, subdivision code 522-043, registered at Nizhny Novgorod region, Navashino city, Pochtovaya str., 1, apt. 122) and/or Koneva Anna Vladislavovna, born on 05.08.1970 (passport 2203 752225, issued on 11.10.2003 by the Department of Internal Affairs of Nizhny Novgorod district Nizhny Novgorod city, subdivision code 522-005, registered at Nizhny Novgorod, Nesterova str., 33, apt. 16)
We grant Trapeznikova A.S., Sosulnikov D.A., Pronin P.V., Koneva A.V. the right to resolve all issues during the trip in accordance with Article 22 of the Federal Law of the Russian Federation “On the procedure for leaving the Russian Federation and entering the Russian Federation”.
The adoption or detention of Ivanov Peter Ivanovich in Hungary is not provided for.

Dear project participants and their parents,
By 05.06.2013, you need to send scanned copies of documents and electronic text versions of completed forms to DASosulnikov@rosatom.ru and pronin@nuckids.ru. Please note that the whole set of documents should be archived and sent in ONE letter to avoid confusion. Please name the archive with your child’s name.

The package of documents you have collected for Schengen visa should be sent by 10.06.2013. Send it by EMS Russian Post (www.emspost.ru/ru), DHL (www.dhl.ru) or any other courier service to the following address:
119017, Moscow, 24 Bolshaya Ordynka St., Rosatom State Atomic Energy Corporation to the name of S.G. Novikov, Director of the Communications Department.
We do not recommend sending documents by Russian Post, due to the increasing number of cases of loss or delay of documents.
If it is possible to transfer the documents by other means (in person, through colleagues or relatives) you should come to the address: 24 Bolshaya Ordynka St., Moscow (Tretyakovskaya metro station), having previously informed Sosulnikov Denis by phone +7 499 9494746.
Documents that the child must have with him upon his/her arrival to Moscow:
1. A Russian passport or birth certificate with an insert or a mark of citizenship of the Russian Federation (original).
2. Health insurance policy (original document)
3. Medical insurance for traveling abroad. You can issue in any insurance company. For the medical insurance, specify the current terms of the child’s stay outside the Russian Federation, that is, from July 6th to August 5th, 2013.
4. A completed and certified medical record (you can download it here).
5. A certificate of epidemiological safety (taken three or less days before departure at the polyclinic at the place of residence)
ATTENTION! If the child ALREADY HAS a valid Schengen visa in his passport, which allows entry into the Republic of Hungary, PLEASE INDICATE THIS!
