Documents that a child must have at the time of his or her arrival at the International Creative Project for Children Nuclear Kids 2017
1. Consent for a minor to travel abroad (NOTARIZED).
NECESSARY CLARIFICATION. The document must be signed by BOTH parents!
In the absence of a second parent, you must provide:
– a court decision on guardianship;
– in case of death of the second parent – a certificate of his/her death;
– if the whereabouts of the parent is unknown – a certificate from the police.
If the parents are not the legal guardians, a certificate confirming this is additionally required. This is a requirement of the Hungarian Consulate for visa processing.
If possible, please use the form we have enclosed. It contains formulations that will eliminate the need for a separate Power of Attorney to submit documents and obtain a visa at the Hungarian Consulate, the need to obtain the Consent of a legal representative to travel with a minor accompanied by adults and the Consent of a legal representative to provide medical care. In case the Notary refuses to execute the document according to our form, you should try to keep the clauses about traveling within Russia and about medical treatment. Otherwise, you should issue a standard Consent for a minor to travel abroad, while it is necessary to issue additionally:
– A power of attorney to submit documents for obtaining a visa at the Hungarian Consulate for the persons indicated in the Consent for a minor to travel abroad (NOTARIZED);
– The consent of the legal representative for the trip of a minor accompanied by adults (simple written form);
– The consent of the legal representative for the provision of medical care (simple written form).
2. Application form of the Hungarian Embassy. YOU DON’T NEED TO FILL IT OUT! Signatures of BOTH parents are required in four places with a BLUE pen.
3. Two photos.
– size 3.5 x 4.5cm, color, clear, clean and contrast,
– the photo should be printed on high-quality paper on a light background,
– the photo should be not older than 6 months,
– made full-face, so that the face occupies 70-80% of the photo.
If the applicant is wearing glasses, then the photo must also meet the following criteria: the glasses are not darkened, the frame should not cover any part of the eye, and there should be no glare.
4. A foreign passport, the validity of which ends no earlier than January 2018. Two free pages must be available
5. A general civil passport of a citizen of the Russian Federation or a birth certificate with an insert or a mark of citizenship of the Russian Federation.
6. Certificate from the place of study (with the seal and signature of the responsible person). The certificate of the educational institution shall contain the address, phone number of the school and student’s class.
7. Medical insurance for traveling abroad. It is issued in any insurance company. For the medical insurance, specify the current terms of the child’s stay outside the Russian Federation, that is, from August 9th to August 15th, 2017.

9. Completed and certified medical record according to the project form
10. A certificate of epidemiological safety (taken three or less days before departure at the polyclinic at the place of residence)
11. A copy of the civil Russian passport (all completed pages shall be copied).
12. A copy of the birth certificate (even for those minors who have already received a passport).
13. Copies of Russian civil passports of BOTH parents (all completed pages shall be copied)
