Documents required for the project

Documents that a child must have at the time of his or her arrival at the International Creative Project for Children Nuclear Kids 2019

1. A foreign passport with an open EU visa (A CRITICALLY IMPORTANT DOCUMENT, the original).
2. Passport/birth certificate (as original document).
3. Consent for a minor to travel abroad (NOTARIZED). Consent is best done according to the proposed form. It is mandatory to list the accompanying persons indicated in the proposed form. (CRITICAL DOCUMENT, original)
4. The consent of the legal representative for the trip of a minor accompanied by adults (Simple written form);
5. The consent of the legal representative for the provision of medical care (Simple written form).
6. Medical insurance for traveling abroad. It is issued in any insurance company. For the medical insurance, specify the current terms of the child’s stay outside the Russian Federation, that is, from July 5th to August 3rd, 2019 (A CRITICALLY IMPORTANT DOCUMENT, the original). ONCE AGAIN! EVERYONE MUST HAVE WITH THEM THE INSURANCE FOR THE PERIOD OF STAY ABROAD THAT YOU OBTAINED FOR YOUR VISA.
7. Health insurance policy.
8. A completed and certified medical record according to the project form.
9. A certificate of epidemiological safety (taken three or less days before departure at the polyclinic at the place of residence)
10. Application for participation in the project
11. Consent to the processing of children’s personal data
12. Consent to photo and video shooting of children.

ALL of the above documents are required for the project participants. If one of them is absent at the time of arrival, the project coordinators have the right to refuse further participation. Especially important ones (I have highlighted those ones without which the trip simply will not take place).

During the project, the documents of each child are placed in a separate file and stored with the responsible person. They are handed out only during transportation and if necessary.
