Trapeznikova Anna Sergeyevna, born 20.7.1961, passport office: 20 0338370,
issued by the Russian Foreign Ministry, the 16.03.2011, registered address:

According to Art. 22 of theFederal
Law ” The Procedure of departure from the Russian Federation and Entry
into the Russian Federation” agree that Amoz Vyacheslav and / Anna S.
Trapeznikov take responsibility for the lives and health of my child and to
take all urgent solutions to protect the rights and interests of my child,
including medical issue, as well as file any request and receive any documents.

Adoption or delayed Medvedev Ilya Dmitrievich in the departure
country is not envisaged.
