1. Documents that will be required for arrival at the International Children’s Center Orlyonok:
Medical card according to the template of the Children’s Center Orlyonok (download here http://www.center-orlyonok.ru/zip/karta.zip)
A copy of the birth certificate and citizenship insert (for children up to 14 years old) or a copy of the civil passport (page with photo and place of residence).
A copy of a medical insurance policy
Epidemiological certificate (to be obtained 3 days or less before departure at the clinic at the place of residence).
The voucher form for the Orlyonok Children’s Center (download here http://center-orlyonok.ru/putevka/put.zip), signed by one of the parents with a BLUE ballpoint pen. NOT A PHOTOCOPY! Original only! The form is not a voucher. In this case, it is important to familiarize the legal representatives of the child with the rules of stay in Orlyonok and agree with them, expressed by signing the form.
Please note that the presence of all these documents is a MUST! In the absence of even one of them, the child cannot be accepted to the Orlyonok Children’s Center and will be sent home.
2. Documents required for participation in the International Creative Project for Children NucKids 2012:
Birth certificate with an insert on citizenship of the Russian Federation or a Russian civil passport (original).
Foreign passport of the Russian Federation citizen with validity period not earlier than March 2013 (original).
Medical insurance policy (original).
Notarized consent of both parents for the child’s departure from the Russian Federation
3. Documents required for obtaining a visa to Hungary (they must be prepared by July 2nd)
The application form (to be filled out in Russian)
The application form should be on one sheet, a double-sided copy (the application form should not be filled in, both parents should sign it)
Two photos 35×45 mm (color, matte, on white background, face 80% of the photo, without corners and ovals). The photos must be new and not used for other visas.
A copy of the Russian passport (all pages), an original and a copy of the birth certificate.
Birth certificate original and copy (regardless of Russian passport)
A foreign passport valid for 3 months after the visa expiration date. Presence of two blank pages is obligatory (original + copy of all pages).
Canceled or old foreign passport, and if there is a second valid one with Schengen visas.
Certificate from the place of study
Financial guarantees of cash at the rate of 50 EUR per day (copy of credit card + bank statement + sponsorship letter or traveler’s checks).
Consent of both parents original and notarized copy + copies of parents’ Russian passports (Notarized consent of both parents for the departure of the child to the Republic of Hungary in the period from August 10th to August 25th, 2012 as part of a group led by Amosov Vyacheslav Alexandrovich (Passport series 2202 № 322181 issued by Bolshemurashkinskiy Regional Department of Internal Affairs of Nizhny Novgorod region on August 26th, 2002. Subdivision code 522-024. Registration: Nizhny Novgorod region, Bolshemurashkinsky district, Chernukha village, Novaya street, 12 apt. 2).
The consent MUST INCLUDE THE GROUP LEADER, COUNTRY AND DATES OF STAY! We have purposely extended the dates of stay in Hungary. This is because negotiations with the host country are ongoing and the dates of our group’s visit may be adjusted.

It is necessary to collect documents for obtaining a Schengen visa. The full package of documents in the original must be prepared by July 2nd, 2012.
